Your Journey to Health
Complimentary Consultation - I would love to have a conversation with you to discuss your goals and allow us to get to know each other a bit. I want to ensure that your expectations are met. This is an optional step, but it is highly encouraged. Consultations can take place over the phone, through Google, FaceTime or in person.
Step I: Comprehensive Health History Questionnaire - to be completed and returned prior to your initial assessment visit. I highly recommend you also send me any medical records and laboratory results from the last 3 years. Reviewing all your health information prior to the initial assessment allows me to have a solid background on your case so we can hit the ground running at your first visit.
Step 2: Initial Assessment – a review of your history, goals, current state of health and a clarification session. We will discuss my review of your records, perform a physical assessment as needed and develop a strategy for your plan of action.
Step 3: Plan of Action - Following your assessment session, you will receive via email, a summary of our conversation and specific steps to take to achieve your goals. This will include:
· An Individualized Focus on Whole Foods
· Nutritional Supplement Schedule Changes (if necessary)
· Movement and Exercise Considerations
· Potential Laboratory Tests or Other Assessments
· Key Areas to Monitor
· Resources and Ways to Maximize your Plan
Step 4: Follow Up Visits – Review progress achieved through action plan, discuss any modifications or concerns, re-assess and compare lab values to ensure you are on track to achieve your goals.
The number of follow up visits and length of treatment plan will vary on an individual basis. Most cases may require only a few follow ups while more complex cases may require up to 8-12 follow up encounters.
Keep in mind, it may take up to 3-6 months to detect measurable changes in laboratory findings as the body regenerates and replenishes stores of nutrients or adapts to sustained physiologic changes. Follow up tests and visits will be scheduled as to allow reasonable time for the treatment plan to take effect.
